Facebook doesn’t need to dazzle these users with Home or some other contraption. Facebook simply needs to maintain its comfortable experience for existing users while adding in functionality that takes off elsewhere, like Instagram Video (copying Twitter) or check-ins (copying Foursquare).While some numbers have suggested a teenage exodus off the platform, the truth is more complex.Facebook’s Willingness To Bet (And Buy) BigAnd when a rival service shows enough growth with teens or other desirable demographics to warrant it, Facebook can simply acquire the product. While the company already dominated photo sharing, Facebook acquired Instagram to give its mobile photo business a jumpstart. Facebook can afford to buy out rivals, given the mountainous cash hoard it raised in its IPO.This is why I’m sanguine on Facebook’s future. The social networking giant is in the perfect position to survey the market for rising innovations and blend them into the existing Facebook experience in ways that complement the comfortable Facebook experience. Usually this can be done through home-grown development, but when an acquisition is needed, the company has shown the willingness to spend big on a category leader.This is in stark contrast to another market leader that has generally embraced the embrace-and-extend approach to innovation: Microsoft. As The Wall Street Journal‘s Rolfe Winkler puts it, “Microsoft keeps hitching its fortunes to lame horses,” turning to Nokia, Yahoo!, Dell, Barnes and Noble and other great companies whose market presence has faded. So long as Facebook is willing to clone or acquire the best rival innovations, it should be able to continue growing its revenue even as it improves its utility to new and existing users. Facebook increasingly plays copycat to Twitter and other Silicon Valley upstarts when it comes to innovation, but with over 1.1 billion active users, Facebook can afford to be a fast follower. Indeed, the right strategy may well be to simply copy or acquire others’ innovations as they become popular. After all, as much as people may wring their hands about Instagram or Snapchat beating Facebook to this or that feature, the hardest feature of all to copy is a massive, built-in audience. A Quick History Of Facebook Imitations (Er, “Innovations”)A quick review of Facebook’s most recent product updates suggests that the company is having trouble coming up with novel features. Hashtags are the latest feature to arrive, a clear rip-off of Twitter, and just before that Instagram Video, which looked suspiciously like Twitter’s Vine. Or how about its check-in feature, which mimicked Foursquare? While some might pillory Facebook for lacking creativity, the fact is that being a fast (or even slow) follower is arguably the right strategy for a company with such a deep installed base. When was the last time you or your friends actually celebrated Facebook changing its interface or otherwise altering your comfortable Facebook experience?And did anyone really want Facebook Home? This counts as real innovation and it also counts as a serious innovation misstep.It turns out that copying others’ functionality is not only safe, it’s smart. Just ask Apple. Apple hasn’t innovated the portable music player, smartphone or tablet. Yet it still dominates either market share or profit share in all of these markets by improving upon others’ innovations. Predicting the future is really hard and frankly doesn’t usually lead to big paydays. But making that future safe and easy for a large audience? That pays big dividends.Facebook’s Numbers Suggest It’s Doing Things RightWhile Facebook has a lot of work to do on its monetization strategy, and its advertising still leaves something to be desired, as Hunter Walk points out, the company continues to print money. Critics howled that the company didn’t “get” mobile, but each quarter the percentage of Facebook’s company’s revenue derived from mobile keeps growing, most recently jumping to 30% in its Q1 2013, up from 23% a year ago. Antibiotic levels in water and potential risk of drugresistant bugs

(Editor)Mar 27 2019A JRC report brings together data on antibiotic levels in water, showing that small concentrations have found their way into a range of Europe’s waterbodies.Because of the threat of new drug-resistant bugs evolving when these antibiotics come into contact with bacteria present in the water, scientists are gathering evidence to better understand the potential risk.Their data confirms that the levels of antibiotic residues in drinking water are minute and do not represent a risk to human health.However, antibiotic residues can be found at higher levels in waste water, surface waters, agricultural runoff and water used for aquaculture (farms of fish, muscles, seaweed and other marine species).The report is part of JRC efforts dedicated to investigating the implications of antibiotics in water.Scientists aim to determine the minimum concentration of antibiotics that could cause resistance in bacteria, so that future limits can be based on risk assessments that take into account this potential.The report also highlights that the development and spread of antimicrobial resistance (AMR) (the phenomenon of bacteria resisting the effects of antibiotics) can be constrained if measures are taken to improve the effectiveness of wastewater treatment processes and to control the use of antibiotics in medicine and animal husbandry.Where do antibiotics end up after they’ve done their job?Antibiotics are prescribed for a vast range of bacterial infections in humans and have saved the lives of millions since their discovery.They are also given to animals as part of veterinary treatment, including to control infections in farm livestock.Resistant bacteria regularly evolve in places where antibiotics are commonly used – such as in hospitals where the MRSA ‘superbug’ (resistant to a wide range of antibiotics) is often found.On top of this, antibiotics don’t simply disappear after they’ve done their job of fighting off a bug.They are excreted from the body and so there’s also a risk of similar bugs proliferating in water in treatment plants, in manure and slurry, and in the environment if the concentration of antibiotics is high enough to select for their survival.The prevalence of antibiotic use has led to growing concern over the spread of AMR. In Europe, about 25000 people die of infections from antimicrobial-resistant bacteria every year.It’s also estimated that AMR costs the EU €1.5 bn per year in healthcare costs and productivity losses.Which antibiotics? How are they monitored?Looking at data on 45 antibiotics from 13 countries worldwide, the report’s authors found sulfamethoxazole, trimethoprim and ciprofloxacin to be the three most frequently found in the water that flows out of waste water treatment plants.Most of the data came from Europe (79.2%) and, for the antibiotics detected, the concentrations ranged up to 1 µg/L (one millionth of a gram per litre).Related StoriesA bacterium may limit cardiovascular risks of 1 in 2 people, study showsRaw meat can act as reservoir for bacteria associated with hospital infections’Scissors’ component of CRISPR/Cas9 sometimes gets stuckThese medicines are all commonly prescribed for urinary tract infections, while ciprofloxacin is also prescribed for ear and chest infections.Similar concentrations of antibiotics were also reported in surface waters, despite the fact that a reduction in their levels might reasonably have been expected due to the dilution of these substances as effluents run into rivers and lakes.Ciprofloxacin is included alongside amoxicillin, erythromycin, azithromycin and clarithromycin on the Watch List monitoring program under the EU’s Water Framework Directive, which gathers data on substances that may pose a risk at EU level.The analytical methods used have to be able to detect concentrations as low as between 0.019 and 0.089 µg/L, corresponding to concentrations considered not to have direct effects on aquatic organisms such as algae, crustacean and fish.Antibiotics in fish farms?The report notes that therehasso far beenlittle research into the use and effects of antibiotics in the aquaculturesector.In Europe,the use of antibiotics inallanimalfarming activities, including aquaculture,isregulatedby specific legislation.Aquaculture products(as well as any products from the animal farming systems)must not contain pharmacologically active substances abovean established Maximum Residue Limit.Efficient monitoring at EU levelreliesonsurveillance programmesimplementedin the EU Member States, andrelevant dataaremadeavailableto the Commission.Aquaculture is the fastest growing food-producing sector and it is estimated to account for approximately half of the total food supply coming from fish.While it is necessary to prevent bacterial diseases in aquaculture products, the use of vaccines could lessen the need for chemicals and antibiotics in this sector, on condition that vaccines against the most relevant diseases are registered and accessible in all EU member states.BackgroundIn June 2017 the Commission adopted the EU One Health Action Plan against AMR. The key objectives of this new plan include:1. Making the EU a best practice region2. Boosting research, development and innovation3. Shaping the global agendaThe Commission has also adopted the first deliverables of the plan, for example the EU Guidelines on the prudent use of antimicrobials in human health.The guidelines aim to reduce inappropriate use and promote prudent use of antimicrobials in people. Promising methods for early detection and treatment of rheumatoid arthritis

(Editor)Jun 10 2019Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disorder that leads to significant health issues as well as high treatment costs. In this themed issue of Clinical Therapeutics, published by Elsevier, experts review multiple aspects of RA detection and intervention with the overall goal of moving the field closer to developing effective preventive measures. Identifying people before they develop the disorder could significantly alter the course of disease and spare people its damaging effects.RA affects around one percent of the population worldwide. It leads to swollen, painful joints and can also damage other body systems such as the skin, eyes, lungs, heart, and blood vessels. This debilitating disease results in diminished quality of life, loss of work, pain, and suffering. It is also largely a “forever” disease from which patients with full-blown RA will suffer for the rest of their lives. While medications can control RA for many patients, very few experience a complete cure and are able to discontinue treatment. RA is an expensive disease. In the United States it currently costs around $20,000-30,000 per patient annually for treatment.Guest-edited by Kevin D. Deane, MD, PhD, Associate Professor of Medicine, Division of Rheumatology, Department of Medicine University of Colorado Denver School of Medicine, Denver, CO, USA; and Tsang Tommy Cheung, MBBS (HK), Clinical Assistant Professor, Department of Medicine, LKS Faculty of Medicine, The University of Hong Kong, Hong Kong, this themed issue taps into the expertise of many scientists across the world and discusses multiple aspects of RA prevention. “These discussions will hopefully provide insights into how we can move RA forward to the point where we are preventing disease and also give guidance on how other autoimmune diseases could be prevented as well,” explain the Guest Editors.Many studies are already underway to learn how to prevent RA, however, prevention of autoimmune diseases is still new territory and there is a lot to discuss and learn. “Most people are familiar with prevention for diseases such as diabetes, heart disease, or cancer,” notes Dr. Deane. “For example, it is very common for someone to have routine blood tests, which might reveal high cholesterol, a potential risk factor for a future heart attack. That individual can then implement lifestyle changes like more healthful eating, smoking cessation, and more exercise, or taking a medication to lower the risk of a future heart attack. We developed these approaches for heart attack prevention through clinical trials. The RA community has learned from these approaches and similar prevention-type trials are now underway in RA.””Most autoimmune diseases are only identified once an individual gets ‘sick.’ For example, with RA, once someone has painful, swollen joints,” adds Dr. Cheung. “Blood-based tests can now identify individuals who are at risk before they feel sick, opening a whole new world of screening and possible prevention. Treating RA very early may allow for cheaper, safer therapies to work because once full-blown RA has developed, typically very powerful medications are needed to control disease.”In a Commentary on the clinical burden of RA, John M. Davis III, MD, MS, Associate Professor of Medicine, Division of Rheumatology, Mayo Clinic College of Medicine and Science, Rochester, MN, USA, makes the case that preventive approaches would greatly benefit RA patients. “There have been great advances in the development of conventional synthetic, biologic, and targeted disease-modifying antirheumatic drugs (DMARDs) to treat RA as well as strategies to use these agents to control disease-associated inflammation to the state of either low disease activity or clinical remission,” he comments. “However, with any given treatment strategy, up to 40-60 percent of patients ultimately respond inadequately. All of the authors contributed in a significant way to the overall picture of how RA prevention is developing.The issue identifies several important challenges: Finding individuals who are at-risk for future RA through simple methods, which could be population-based blood testing or other approaches. Getting the research and medical community to agree on the right terminology for RA. “Right now, RA is only applied once someone has arthritis. But it may be that we need to develop new terms like ‘Pre-RA’ that can be used to identify someone at high risk for future RA. For example, the term pre-diabetes is commonly used and is helpful for people to understand that they are in a stage of disease that indicates that they are at risk for getting worse unless they do something. We need similar terms in RA that can resonate with people, and help them take action,” comments Dr. Deane. Editor-in-Chief Richard Shader, MD, Tufts University School of Medicine, Boston, MA, USA comments: The efforts of this team of experts to raise awareness of RA and to explore methods for early detection and intervention should catalyze the medical and scientific communities to increase their efforts to find better ways to treat and perhaps even prevent RA and its complications.” Many other rheumatic and autoimmune diseases follow a similar model as RA where there are blood markers that are abnormal, sometimes years before an individual feels “sick” from disease. These diseases include lupus, gout, and others such as vasculitis. Getting society to invest in prevention, which requires that groups such as governments, the insurance industry, and pharmaceutical companies are also interested in prevention and willing to support it.center_img RA science is in a fortunate situation compared to many other inflammatory diseases where it is rarely known when and where disease-specific immunity may be triggered and how it may gradually evolve towards targeting of the end organ. Research and solutions proposed in this issue may also serve as a demonstration example for many other chronic immune-mediated diseases.”Lars Klareskog, MD, PhD, Rheumatology Unit, Department of Medicine, Karolinska Institutet and Karolinska University Hospital, Stockholm, Sweden, in a Guest Editorial Prior research has already shown that the development of RA can be delayed with a single dose of medication that is typically used in people who have full-blown RA. That finding suggests that if individuals can be identified at the right time, future RA can be delayed or completely prevented. There are also multiple clinical trials underway that should help determine which drugs have the potential to prevent RA and who the best candidates are to receive this treatment.”Treating RA very early may allow for cheaper, safer therapies to work because once full-blown RA has developed, typically very powerful medications are needed to control the disease. This is like stopping a fire when it is still at the stage of a candle – pretty easy. However, stopping a fire once a full-blow forest fire has developed is very hard!” conclude the Guest Editors. Patient preference is also a major challenge. “Asking at-risk individuals to take medications with possible side effects when there is no clinically apparent disease is not easy at all,” observes Dr. Cheung. Bikeshare companies are transforming US cities – and theyre just getting started

The city could soon have one of the largest bike-share systems in the country. Cities around Boston that are outside of the service area of Hubway, the area’s public bike-share system, just reached a deal to provide dockless bike-share service, expanding access to hundreds of thousands of people. And in San Francisco, Uber recently purchased Jump Bikes, a dockless electric bike-share startup, and soon will allow users to reserve electric bikes with their Uber app. If recent examples are any indication, bike sharing in the United States will be a mix of complementary dock-based and dockless systems, run by both public entities and private companies. The humble bicycle, aided by smartphone technology, is resurging as an urban transportation option. Officials in Dallas warn dockless bike-share companies to keep bikes from blocking sidewalks, ramps and trails. Dockless systems are also helping to address equity issues posed by public dock-based systems, which often are located in more affluent and predominantly white urban neighborhoods. Because dockless systems don’t require stations, they can be rapidly deployed in zones that dock-based systems may be slow to reach. Students at Beijing University developed this approach in 2014 to improve campus mobility. Dockless bike-share companies have flooded Chinese cities with bikes in the past two years, leading to massive piles of discarded bicycles in public spaces. The modern concept of bike sharing – offering bikes for short-term public rental from multiple stations in cities – was launched in Copenhagen in 1995, but U.S. cities only started piloting their own systems in the past decade. Washington D.C. led the way, launching SmartBike DC in 2008 and an expanded network called Capital Bikeshare in 2010. This program now boasts over 480 stations and a daily ridership of 5,700. Within a few years, bike-share systems launched in Boston, New York, Chicago, San Francisco, Seattle and dozens of other cities. In 2016 there were 55 systems across the country with a total of over 40,000 bikes. And momentum continues to grow. In 2017 Citi Bike in New York City added 2,000 bikes, increasing its fleet to a total of 12,000. San Francisco is expanding its system from just 700 bikes to 7,000, thanks to a sponsorship deal with Ford. Going docklessThe newest twist in this rapid expansion is dockless bike sharing, which lets users park bikes anywhere within defined districts and lock and unlock their bikes with smartphone apps. Users don’t have to locate docking stations or worry about whether space will be available at their destination. These systems also are cheaper to set up, so providers can charge lower user fees. Some dockless bike-share companies offer rides for as little as US$1 for the first half hour. Bake in centre rack of the oven for 8 minutes until top is lightly golden.7. While the cake is baking, place parchment paper on a damp towel and dust with icing sugar.8. When the cake is ready, remove from oven and turn over onto the dusted parchment.9. Peel off bottom parchment and trim away any jagged edges if necessary.10. While the cake is still hot, pre- roll cake to prevent cracks.11. Unroll the cake and spread kaya all over it.12. With the help of the parchment paper, roll cake again into a tight coil. Keep cake wrapped in parchment to cool.13. Cut into 2.5cm (1 inch) slices and serve.
